The Community RN of L’Arche St. Louis is responsible for monitoring the health and well-being of individuals receiving residential care (“core members”) at L’Arche St. Louis. The Community RN should get to know each core member, be familiar with their medical history, and stay up-to-date on their overall health including current medications, diagnoses, and ongoing concerns. The Community RN is responsible for staying informed of current rules and regulations pertaining to their role as outlined by the Missouri Department of Mental Health.
The Community RN role is supervised by the Assistant Director and maintains a close collaborative relationship with the Assistant Director and the House Leaders of each of our 3 homes in managing the medical needs of the core members. The Community RN is part of each core member’s care team which includes healthcare providers, L’Arche leadership, and family members and guardians to ensure continuity of care.
Duties:
Oversight of medical care coordination:
- Monthly face-to-face assessment. Document findings in Monthly Health Summary. Additional assessments as needed including after hospitalizations or illness.
- Monitor Medication Administration Records on a monthly basis and after any major medication change.
- Availability during off-hours by phone/text or email for guidance on non-emergency medical issues (this is not an “on-call” position).
- Occasional visits as needed to the home, rehab facility, or hospital. Attend ISP meetings and doctors’ appointments as necessary to address a specific need.
- Ensure team is trained on new and ongoing medical issues and feels confident in providing care. Identify and address any deficiencies in the standard of care provided by the assistants
- Annual AIMs evaluations for individuals with that identified need.
Oversight of direct care staff in the following areas (with support from the Assistant Director):
- Oversight of all function of medication administration by medication aides certified through DHSS including but not limited to:
- Review of physician orders, Medication Administration Record, and staff documentation.
- Periodic inspection of technique and skill of certified aides delegated to pass medications.
- Overseeing staff (assistants) in the delivery of delegated nursing care tasks.
- Identify need for staff supervision/delegation or specialized instructions and any follow-up place of action that may result.
- Ensure that delegation/specialized instruction is provided and documented
- Ensure periodic oversight/supervision of staff delegated in tasks
- Ensuring timely and accurate documentation of current physician orders, annual PPD and other routine immunizations, controlled substances, and others as required by DMH.
Accountability for activities:
- Complete Monthly Service Log to accurately record time dedicated to the functions of the Community RN Program as defined by DMH.
- Ensure total hours for each month are accounted for and do not carry over to the next month.
Maintain communication with L’Arche Leadership Team, House Managers, and Guardian, Families including changes to the individual’s health status and identifying new needs.
Required training and certification:
Must be a registered nurse with a current license in the state of Missouri.
Community RN Training provided by Missouri Department of Mental Health (provided by L’Arche)
First Aid & CPR (renewals provided by L’Arche)
Qualifications:
Experience or familiarity with any of the following conditions:
- Adults with intellectual/developmental disabilities
- Down Syndrome and other chromosomal disorders
- Mental and behavioral health
- Aging and geriatric medical issues
- Palliative care and hospice
Able to learn and use new software for medical care coordination and documentation.
Hours and Availability:
Part-time position of approximately 30 hours per month. Days and times are flexible.
